HEALTH CASE MANAGER
If you are a Registered Nurse, in Manitoba, we would like you to become part of our Drug Services team. As a Health Case Manager: Your primary focus is working with our Plan Members to act as an advocate and liaison to meet a member's individual health care needs by assessing, planning, implementing, coordinating, monitoring and evaluating options and services. Serve as a key contact for members and work collaboratively and communicate clearly and professionally with physicians, providers, co-workers and other members of the Drug Services team in order to carry out the established plan of care. Through telephonic health coaching and education, you will play a role in determining whether the drug treatment prescribed is reasonable opposite an identified medical condition and the contractual parameters. You will serve as a strategic resource for our Prior Authorization Drug program, and provide first level support for our Drug Services team. You have a working knowledge of available community support programs within designated regions of Canada.
Your Qualifications and Competencies: A Bachelor Degree in Nursing with a current license from the applicable Provincial licensing body. Current registration with the College of Registered Nurses of Manitoba and a minimum of 4 to 7 years of related experience. Applied medical and case management skills. Demonstrated leadership ability with comprehensive knowledge of nursing and health care / case management practices and techniques. Strong assessment, planning, implementation and evaluation skills. Demonstrated ability to handle multiple demands under strict time constraints. Strong written and verbal communication skills; preference will be given to French bilingual candidates. Solid interpersonal skills and ability to work well within a team. Excellent problem solving and multitasking skills.

The North Peel Family Health Team (NP FHT) brings together four family doctors, a nurse practitioner, registered nurses, a social and mental health worker and a dietitian, as well as a pharmacist, a psychiatrist and support services, to provide comprehensive interdisciplinary primary care for NP FHT patients. Operating within your full scope of practice as a Nurse Practitioner in this full-time, permanent position, you will work collaboratively with all FHT team members in treating identified illnesses, promoting patient health and preventing disease.
As a Nurse Practitioner, you will:
• Conduct periodic examinations and risk assessments that may include care of at-risk populations, wellness exams, pap tests and breast exams, pediatric care, well-baby visits and immunizations
• Provide care to patients with specific intermittent illnesses
• Collaborate with other multidisciplinary team members and external partners
• Develop and conduct a broad range of health promotion and chronic disease management processes, programs, and patient education (group and individual) in collaboration with multidisciplinary team members
• Provide input for the development of the FHT’s Program planning
• Maintain accurate, concise and confidential EMR documentation pertaining to patient history and encounter data
• Complete necessary FHT Program business documentation
• Communicate ideas and concerns to the team and participate in problem-solving
• Assist in new staff training and development
• Participate in educational sessions held by other multidisciplinary team members
• Teach and mentor team members on the role of the Nurse Practitioner.
Your profile will include:
• Current registration with the College of Nurses of Ontario as a Nurse Practitioner – Primary Health Care
• Excellent health assessment and clinical skills
• CPR certification
• Working knowledge of computer systems and applications
• Excellent verbal and written communication skills
• Ability to work collaboratively as a member of the FHT’s multidisciplinary team.
• Experience working in a primary care setting would be considered an asset.
• Health promotion skills, including population health and stress management, an asset.
