This tool was developed under the clinical leadership of Dr. Arun Radhakrishnan, and has been designed for day-to-day use in a typical office visit. It has been designed to guide primary care providers in developing and implementing a management plan for adult patients with chronic non-cancer pain in the primary care setting.

This tool has been developed by the Centre for Effective Practice in collaboration with the Ontario College of Family Physicians and Nurse Practitioners' Association of Ontario as part of the Knowledge Translation in Primary Care Initiative, which is funded by the Ontario Ministry of Health and Long-Term Care.
